Guide (FSG). It provides you with information about PetSure to help
you decide whether or not to use the financial services that we provide.
This FSG explains how we can assist you, who we represent, how we
are remunerated, and provides details of how any complaints will be
dealt with as well as how we can be contacted.
You will receive a Product Disclosure Statement (PDS), which explains
the significant features of the products before we issue the product.
You should consider the PDS in deciding whether to purchase the product.

Insurance Company Pty Ltd (Hollard), A.B.N 78 090 584 473
About The Hollard Insurance Company Pty Ltd?
Hollard issues the pet insurance products administered
by PetSure. Hollard is an insurance company authorised under s12 of
the Insurance Act 1973. Hollard holds an Australian Financial Services
License (No.241436) and is authorised to deal in and provide financial
product advice in general insurance products. You can contact Hollard

Who is PetSure (Australia) Pty Ltd?
PetSure is an Authorised Representative No 268991
of Hollard and is authorised to deal in pet insurance products and
to distribute this FSG. PetSure issues these products on behalf of
Hollard and has authority to issue, vary and cancel the products.
PetSure issues the pet insurance products under a binder from Hollard.
This means they act on behalf Hollard and you should separately consider
the relevant PDS to determine if the product is right for you.
Remuneration
PetSure receives a portion the underwriting profit
for administering the pet insurance products issued by Hollard. Where
a third party has referred you to us, they may receive a referral
fee from Hollard. This fee is paid according to Hollard’s agreement
with them and the role they play. It may be a commission or an agreed
amount.
How do we resolve complaints?
In the unlikely event that you have a complaint about
the service provided please contact us by phone (02)9842 4800 or
by writing
If your complaint remains unresolved, PetSure’s dispute committee
will review the matter and respond within 15 days.
Your complaint will be dealt with fairly and promptly
but if you remain dissatisfied with the final decision you may refer
the matter to the Insurance Ombudsman’ Service Ltd (IOS), which
acts as Hollard’s external dispute resolution provider. The
IOS is an independent body and its service is free to you. PetSure
and Hollard agree to accept the Scheme’s decision. To access
the Scheme you must contact the IOS within 3 months of a final decision

Privacy
We use personal information about you to administer
your insurance. We are committed to protecting your personal information
in accordance with the National Privacy Principles. A copy of our
privacy policy is available on request or can be viewed at www.hollard.com.au.
